Nocardial Brain Abscess In A Patient With Myasthenia Gravis

چکیده

Introduction: Infection by Nocardial species is a rare disease, mostly seen in patients with immunocompromised status secondary to malignancy, organ transplant, retroviral infection, or chronic steroid therapy. The most common organ involved is the lung, and primary involvement of the central nervous system is very rare. The following case that we present is of a patient with history of myasthenia gravis on chronic low dose immunosuppressive therapy that presented with altered mental status, and was found to have cerebral Nocardiosis confirmed after brain biopsy, without any pulmonary infestation.

Nocardial brain abscesses remain a clinical challenge. We successfully treated a patient with nocardial brain abscess, mycetoma, pneumonia, and glomerulonephritis. Nocardial soft tissue involvement, mycetoma, is well known. However, the fact that actinomycetoma can metastasize may not be as well appreciated.
